Folio, old vellum; contents browned and dampstained, outer half of p5 excised, last leaf trimmed and mounted, lacks preliminaries up to 2*1 (title, author's preface, and beginning of table of contents, all replaced in manuscript) and everything after 2i6. Np, later 16th-early 17th century
